Over the years, we have had requests for many of these, but they have never been available to depository libraries. This list is not comprehensive, but comprises those forms most asked for by the public. Many other agencies have quite specialized forms and should be accessed directly. Bottom Bureau of Alcohol, Tobacco and Firearms & Exposives This site contains an extensive list of forms for those in the alcoholic beverage, firearms, tobacco, and explosive industries. Alcohol and Tobacco Tax and Trade Bureau The TTB insures proper excise taxing for alcohol & tobacco. Copyright Office This link has informational publications as well as forms. Customs Service Downloadable froms from the U.S. Customs Service. Defense Logistics Agency The DLA provides supplies and services to the military. These forms are for doing business with the military. DLA, DD and SF forms. Must have FormFlow software. Department of Defense Electronic Forms by Form Number An enormous amount of forms in PerForm Pro, Form Flow 2.0, or Adobe PDF! Department of Energy DOE Financial Assistance Forms. Department of Health and Human Services Extensive list of downloadable forms. There is also an interactive form site called Intelligent Forms which uses JetForm Filler (the site for downloading this application is linked). Department of Homeland Security INS Executive Office for Immigration Review Department of Labor Forms from ERISA (Employee Retirement Income Securities Act). A search engine is included. Federal Acquisitions Regulation Forms From the General Services Administration. These forms(SF and OF) are available by a numerical list and alphabetically by title. Federal Communications Commission This site has a huge list. Federal Emergency Management Agency Excellent site containing much important information about federal emergency help, including forms. Food and Drug Administration Extensive collection of forms. FASFA: Free Application for Student Aid Form from the Department of Education Check Browser requirements at the web site General Services Administration Forms library Center for Medicare and Medicaid Services (formerly Health Care Financing Administration) ContainsCMS program forms. Top | Bottom Housing and Urban Development Forms Warehouse Forms are PDF, Excel, or Word Citizenship & Immigration Service (formerly (NS) Many forms are available from this site, as well as a quick site for downloading I-9s. If the form is not available online, there are instructions on how to contact the agency to receive it. Internal Revenue Service Excellent site containing many forms. However, this site is EXTREMELY busy during tax time. Reproducible forms are available in many libraries, including older forms. Military Service and Family History Records Military records along with census and immigration records from the National Archives. National Institutes of Health Office of Sponsored Research National Science Foundation Grant proposal "kits" are available for downloading which include forms. This excellent site also contains manuals for help in writing and preparing grants. Office of Personnel Management Well-organized page offering many different forms federal workers and their families need. Also included here are federal employment forms, including the OF-615. Please check with your agency before submitting an SF-171. Passport Application Forms Contains forms for new passports, as well as renewals. Several other forms are available, too. Patent and Trademark Office This site contains an extensive listing of patent and trademark forms, as well as a few Patent Treaty Cooperation Treaty (PCT) Forms. A more complete listing of PCT forms, including those in German, French, and Spanish, are available from the PCT Legal Office. Postal Service Savings Bond Forms From the Treasury Department, these forms can be used for ordering, claiming, etc. savings bonds. Small Business Administration An extensive list of forms for small businesses including appropriate IRS and OSHA forms. Social Security Administration Forms for application for a social security card are found here, as well as several others dealing with benefits. Several forms are in Spanish. To replace, correct, or change your name on your social security card, start here State Tax Forms from Federation of Tax Administrators This is a commercial site; however, they link to official state sites.
